The following are important dates that will affect your rights:

• Deadline to Submit a Claim Form: If you are a Settlement Class Member whose account is either uncollectible or has an outstanding balance of $75 or less, to receive a share of the Settlement Fund, you must complete and return a signed Claim Form, postmarked, faxed, or uploaded to the settlement website (here) on or before October 24, 2022.
• Deadline to Request to be Excluded from the Settlement Class: You can exclude yourself from the class action and the settlement (or “opt out”) by filing a written request. That request must be postmarked on or before October 24, 2022, and it must list your name, street address, and the name and number of this case.  Directions on how to exclude yourself from the Settlement Class are contained in the Class Notice.
• Deadline to Object to the Settlement: If you object to the settlement, and wish to file an objection rather than excluding yourself, you must submit your objection in writing to the Clerk of the Circuit Court of St. Clair County,
  10 Public Square, Belleville, Illinois 62220. Your objection must be postmarked by October 24, 2022. Directions on how to object to the Settlement Class are contained in the Class Notice.
